EXAMINATION, EVALUATION & TREATMENT PROCEDURE

National Medical Clinic™ physicians participating in the Physician Referral Service of the National Organization of American Physicians™are located in several states within the United States. NMC physicians conduct a prior physical examination, evaluate patient lab tests, perform clinical evaluations of their patients, and obtain a medical indication before the issuance of any prescription for medication to any National Medical Clinic™ patient. This process applies to all National Medical Clinic™ treatment programs described on this site.

NATIONAL MEDICAL CLINIC, INC.
AMERICAN MEDICAL GENERAL, INC.

National Medical Clinic, Inc. (“NMC”) and American Medical General, Inc. (“AMG”) are national patient medical services management corporations with access to member physicians located in a number of states. NMC and AMG contract with physicians, diagnostic medical laboratories, medical laboratory services resellers and other patient service providers to assist patients in securing a wide range of medical services. The cumulative total of the fees attributable to the complete treatment process for a specific medical treatment determines the medical treatment retainer fee payable by a patient. This includes the cost for searching and locating a physician in an area where there is no existing NMC network physician to examine, evaluate and provide medical care to a patient for the treatment sought. The only additional costs that may be incurred by the patient in excess of the basic medical treatment fee for a specific treatment are those additional medication prescribed by the patient's treating physician during the course of treatment when indicated by the patient's response to treatment and the costs for any additional medical laboratory diagnostic tests ordered by the treating physician during the course of treatment.

NATIONAL ORGANIZATION OF
AMERICAN PHYSICIANS™
PATIENT REFERRAL SERVICE

Patients seeking medical treatment through NMC/AMG are referred to physicians who are members of the 'Physician Referral Service' of the National Organization of American Physicians™ (“NOAP™”) in the process of securing patient medical services.

Patient Enrollment and Referral to a Physician

New patient registration with National Medical Clinic, Inc. (NMC) and American Medical General, Inc. (AMG) and request for enrollment In a medical treatment that the patient desires to undergo is conducted by telephone enrollment. No NMC or AMG medical treatment involves the prescribing or use of any controlled substance prescription drug, except testosterone hormone replacement therapy which may be provided by the treating physicians at some locations. If an NMC/AMG member physician qualified to treat patients for the treatment sought by the patient is located within an acceptable reasonable distance from the patient's location, then the patient may register and enroll in the treatment at the time the patient pays the medical retainer fee to AMG by credit card. Patients enrolling in HCG Therapy to Increase Testosterone Production in Men undergo a laboratory blood test to determine if the sought treatment is indicated before enrolling in treatment. Patients register and enroll in an NMC medical treatment program by phoning NMC Patient Services at phone number (800) 582-0095.

Medical Retainer Fee Refund Policy

If the NMC member physician to whom a patient has been referred declines to issue the prescribed treatment sought by the patient after physical examination and evaluation of the patient's medical complaint and condition by the treating physician, the patient's retainer fee shall be refunded less the fees and costs incurred by the patient in obtaining the clinical assessment and physician evaluation as set forth in the enrollment order confirmation emailed to NMC at the time of enrollment.. These costs include the fees and costs for the patient's physical examination, clinical assessment and evaluation by the physician, diagnostic testing and clinic services at emailed to NMC or the patient in the confirmation at the time of enrollment. The patient shall be treated if treatment is indicated and the sought treatment could be beneficial to treating the patient's medical condition.

Legal Compliance

Under federal law, a valid prescription for a controlled substance prescription drug requires (1) a physician’s in-person evaluation of the patient (2) in the usual course of professional medical practice and the (3) issuance of the prescription for a valid medical purpose.. Both the prescribing physician and dispensing U.S. pharmacy must be registered under section 303(f) of The Controlled Substance Act to prescribe and dispense and deliver a controlled substance prescription drug to a patient. State laws governing the issuance of prescriptions for non-controlled substances generally require the following: a medical complaint, medical history, prior physical examination, clinical assessment, logical connection between the medical complaint and any treatment prescribed, issuance of prescription for a legitimate medical purpose in the usual course of professional medical practice.
